At CVS if you purchase 3 Pepsi 12-pack products AND 3 Frito-lay snacks 9.5oz or larger you receive $5 on your ExtraCare card and a $10 cash-rebate by mail from pepsi and frito-lay. This week pepsi products are 3/$9 and frito-lay products are 3/$6 which makes these items FREE. The flyer states the $5 extra bucks will print at the register 2 days after the qualifying purchase has been made. The other form was on the pepsi.

Also CVS will give $10 Extra Care Bucks if you spend $20 on Kimberly-Clark products this week. This includes Huggies and Pull-ups which are on sale this week for $7.99 per Jumbo pack. Use the coupons from last weekends paper and it's an even better deal.

FYI, if you don't want to go to the store and make a purchase just to print out the extra care bucks, you can also print them from their website. They will show up up two days after you make the purchase.

Does the rebate form print out at register or do you find this seperate?

According to the CVS circular,

you could find the $10 rebate ON SPECIALLY MARKED PACKAGES OF FRITO LAY OR PEPSI PRODUCTS or SEPARATELY VIA OFFER IN STORE(TEAR OFF SHEET).

They are the peelie type ones on the packages. There is also a $.25 off cherry pepsi coupon attached to the form and $.25 off Sierra Mist underneath when you peel it off.

My CVS also had a display with a tearpad of the forms next to the shelf.
