Here are a few that I got in the past, times may have changed, so you may want to ask about these:

Olive Garden: Happy birthday song and cake
IHOP: Happy birthday song and cake
Family video: Free rental on your birthday
McD's: If you book a party, the birthday kid gets to eat free
Victoria's secret (I got this yesterday, so it should be still running): Take $10 off any purchase during the month of your birthday. (for anyone who is on their mailing list or has an angel's card)
Noodles company: free dish of noodles
Benningans: Free entree with a purchase of any entree.
Fazoli's: Free dessert

Will edit once I find more.
