A & P Supermarkets' The Live Better Wellness Club gets you extra discounts

http://www.apsupermarket.com/bsc_wellness.asp

Enrollment fee is $4.99 - you then get a coupon good for $4.99 off your next $25 grocery order, so the program is "free".

You join by filling out an application at any of our pharmacy counters, and paying a $4.99 enrollment fee. If you already have a bonus savings club card for our stores, a sticker will be afixed to that card designating your upgrade. You will also immediately receive a sign up bonus coupon good for $4.99 off your next $25 grocery order (redeemed at the regular checkout)…and in two days, your card will be activated to generate the new program’s regular discounts. If you do not have a bonus club card for our stores, you must first apply for one at the Customer Service Counter, and after receiving it, bring it to the pharmacy counter and follow the same process just outlined. The only difference in this instance is that as a new bonus savings club member, you will have to wait five days before beginning to receive the new program’s discounts.

With enrollment, you get:
Storewide Savings on Healthy Living Products
20% off all Health Pride Products
10% off all Fresh Cut Vegetables Every Day
10% off all Fresh Cut Fruit Every Day
Value-Added Benefits
10% Senior Citizen Prescription Discount (60 years or older)
10% Newborn-to-Five-Year-Old Prescription Discount (not valid in NJ)
10% Family Pet Prescription Discount
Live Better Generic Prescription Plan
Pay Only $9.99 for up to a 90 day Supply of Over 300 Generic Drugs

At the link posted above, there's a list of the current offers you get from this program.

You don't have to provide medical info at enrollment, that just gets you personalized info for your condition(s).

To receive personal prescription benefits, each individual patient will need his or her own insurance card. Therefore you would each have to apply for a separate card and pay the $4.99 upgrade fee to get individual prescription coverage.

Edited to add:
Therefore once you have the upgrade your club card is good at all A&P, Waldbaum’s, SuperFresh and Super Foodmart stores.

(The program is at those stores too, although it seems that the products list for the $'s off might differ.)
